Terms of Use
Effective November 7, 2026.
These terms apply to everyone who uses Flourish Four. If your organization has a signed agreement with Form Communities, that agreement controls where it differs from these terms.
By signing in or using Flourish Four, you agree to these terms. If you use it for an organization, you agree for yourself and confirm you are allowed to use it for them.
1. The service
Flourish Four is a records system for Clubhouses and peer-support programs, run by Form Communities ("Form Communities," "we," "us"). Organizations ("your organization") use it to keep records of the people they serve.
2. Accounts and sign-in
- Your organization's administrator creates your login. You can't sign up on your own.
- Keep your password and sign-in links to yourself. Don't share your login, except a shared or kiosk login your organization set up for that purpose.
- Sign out on shared computers. Tell your administrator at once if you think someone else has used your login.
- You are responsible for what is done with your login.
- If you sign in with Google, use the Google account your organization gave you for work.
3. Acceptable use
Use Flourish Four only for your organization's work, and only to see records you need for that work. Don't:
- Look at records about someone without a work reason, including family, friends, or yourself.
- Try to get around access limits, the audit log, sign-in limits or other security.
- Copy, export or share records outside what your organization allows.
- Type member names or private details into feedback, help requests or Ask Flo.
- Upload anything unlawful, harmful or that you don't have the right to share.
- Probe, scan or overload the service, or use automated tools against it without our written permission.
- Use the demo or sandbox sites for real records. They are for fake data only.
4. Your organization's responsibilities
Your organization:
- Is responsible for the records it keeps, and for having a lawful basis to collect, use and share them.
- Gets any consent or authorization the law requires, including for health, substance-use and other sensitive information, and before sharing records with another program.
- Keeps its records accurate and up to date.
- Decides who gets access, reviews it, and turns off logins when people leave.
- Sets up sharing rules, restricted records and access hours to fit its own policies.
- Answers requests from people about their own records.
- Trains its staff on these terms and its own privacy rules.
5. Confidentiality
Records in Flourish Four are confidential. Use what you see only for your organization's work. Don't disclose it except as your organization allows and the law permits. This duty continues after your access ends.
We keep each organization's records confidential and use them only to provide the service, as our Privacy Policy describes.
6. Not medical or professional advice
Flourish Four is a records tool. Its reports, scores, assessments and Ask Flo's answers are not medical, clinical, legal or other professional advice. Decisions about care and support are for qualified people at your organization. Ask Flo can be wrong; check its answers against the help pages and your organization's policies.
7. Availability and changes
We work to keep Flourish Four available, but it may be down for maintenance, updates or reasons beyond our control. Some features can save while you have no signal and send later; check that they sent. We may change, add or remove features. We will tell organizations' administrators before a change that takes away an important feature.
8. Suspension and ending access
- Your organization can change or end your access at any time.
- We may suspend a login or an organization's access if we reasonably believe it is being misused, puts the service or other people's records at risk, or breaks these terms or the law. Where we can, we will tell the organization first.
- When an organization stops using Flourish Four, it can export its records for 60 days, after which we delete them as described in the Privacy Policy.
9. Ownership
Your organization owns the records it puts into Flourish Four. Form Communities owns the software, its design and its help content. You may not copy, resell or reverse-engineer the software, except as the law allows.
Feedback you send us may be used to improve Flourish Four, with no duty to you.
10. Disclaimers
Flourish Four is provided "as is" and "as available." To the extent the law allows, we disclaim all warranties not stated in these terms or in a signed agreement, including fitness for a particular purpose.
11. Limitation of liability
To the extent the law allows, Form Communities is not liable for indirect, incidental, special, consequential or punitive damages, or for lost profits or data, and its total liability for any claim about Flourish Four is limited to the fees your organization paid in the 12 months before the claim, or $100 if it paid none. A signed agreement with your organization controls where it says otherwise.
12. Indemnity
Your organization will defend and hold Form Communities harmless against claims arising from records it collected or shared without a lawful basis or the consent the law requires, except as a signed agreement says otherwise.
13. Governing law and disputes
These terms are governed by the laws of the State of Texas, without regard to its conflict-of-law rules. Disputes go to the state or federal courts in Bexar County, Texas.
14. Changes to these terms
We may update these terms. We will post the new version here with its date and tell organizations' administrators about important changes before they take effect. Using Flourish Four after a change means you accept it.
